As the end of the school year quickly approaches, we wanted to provide several reminders and updates for families! Please come to the health office by June 16th to pick up any remaining medication, Epipens, inhalers, or diabetic supplies that were ordered by your child(ren)’s provider for this school year. Middle and Upper School students may pick up their own medication, with written parental permission, on the last day of school. Otherwise parents must pick up the medication. New medication orders are required for any medication that needs to be administered for the 2017-2018 school year. This includes over-the-counter medication. Pediatric providers may write medication orders on their own forms or the school form, found here.
JV and Varsity Sports begin on August 14th, and modified sports begin on August 30th. In order to participate in fall sports, a health appraisal form, dated on or after August 1st, 2016 must be on file in the health office. Health appraisal forms, which are documentation of a physical exam, may be faxed (585-495-2260) or emailed (email@example.com) to the health office prior to the start of this sport season. In addition, parents must register their son or daughter on FamilyID.
Current health appraisal forms are required for all new enrollees and for returning students in grades Pre-K, kindergarten, 2, 4, 7, and 10. With the exception of the timeline for sports requirement, health appraisal forms dated on or after September 1st, 2016 meet the requirement for returning students.
If your child borrowed any clothes from the health office, please return them by the end of the school year. We are always in need of pants, in good condition, for Lower School children.
